Yes, you can!

To have the survey fully anonymous, and also do not ask for their name and email, you'd need to put in some fake data in the placeholders for your survey. 

For example, you could put [email protected] or something similar on the Customer Email address field so that when the survey comes in, those fields are already filled out, and it won't ask for the information.

To do this, head over to your survey. Click on Edit Survey, and go to the Publish tab. 

Enter the fake data into the Rating placeholder fields:
