Navigate to the Senders tab, click Add > Group members (inside organization) > Select a group. Click on the group that'll be using the email signature survey and click OK.

On the Recipients tab, click Add > External recipients.

Then go to the Design tab > Edit signature > Select the template that you'd like to use or create your own.

Click where you would like to place the customer satisfaction survey. Then, click Insert HTML snippet > paste the Simplesat survey code that you copied and click on OK.

Customize the email signature survey to your heart's design > click Apply & close. Go to the Manage Signatures App, click 💾 to save, and submit changes to your signature rules.

Try it out!
